  2. Fill in the front page of the form

Enclose a check for $500 payable to FLYING MOOSE LODGE

U.S. Funds only $10 fee for returned check

Credit Cards not accepted

Deposit returned for withdraw before June 1, 2010 if reserved space is filled.

After deposit has been received, you will receive:A packing list of required items and suggestionsA packet with information for parents and campers

A Health Form to be completed before camp starts.

In May you will receive more information concerning the first day of camp and a statement for the remaining fee.
